Terms and Conditions for Dyslexia Discovery Assessments

These Terms and Conditions govern the assessment services provided by Dyslexia Discovery. By scheduling an assessment, you agree to comply with and be bound by these terms.
1. Consent and Attendance
- Assessments for individuals under the age of 18 will only be conducted with the consent of parents or those with parental responsibility.
- For individuals under the age of 16, a parent or guardian must be present at the assessment venue.
2. Pre-Assessment Requirements
- The person being assessed must have had a sight test within the last 12 months.
- They must have lived in an English-speaking country and have been speaking English regularly for at least 7 years.
3. Background Questionnaires
- Background questionnaires must be completed by parents and the young person’s school or college. In addition, further interviews or discussions may take place to gather additional information.
- Completed forms must be returned to rosie@dyslexiadiscovery.org no later than 2 weeks before the assessment date.
4. Duration of Assessment
Assessments typically take between 2-4 hours, with short breaks provided as needed.
After completion of the assessment, a report will be aimed to be delivered within 3-4 weeks. A follow-up 20-minute virtual or telephone call will be arranged to discuss the report. Further details will be provided upon booking confirmation.
5. Fees
The total cost of an assessment is £600. A non-refundable deposit of £150 is required to secure your booking, with the remaining balance due 7 working days prior to the assessment date.
If the assessment is being funded by an external organisation (e.g., school, college), an email confirming this funding must be received before scheduling an appointment.
6. Video/Audio Recordings
Permission is requested to video/audio-record sections of the assessment to assist in the marking and analysis process.
The recording will be reviewed solely by the assessor and will not be shared with any third parties. It will be deleted immediately after the assessment report is completed.
7. Limitation of Liability
Dyslexia Discovery strives to provide accurate and comprehensive assessments, but the results are contingent on the individual’s responses and performance during the assessment..
We do not guarantee a specific outcome or diagnosis and reserve the right to decline referrals or discontinue assessments if criteria are not met or if the client fails to cooperate, which jeopardises assessment validity.
A diagnosis of Specific Learning Difficulty (Dyslexia) cannot be guaranteed. Recommendations for existing literacy difficulties will be included whether or not a diagnosis is made.
We aim to deliver reports within the stated timeframe but are not liable for delays caused by circumstances beyond our control. Our liability is limited to the cost of the assessment, and we are not responsible for any indirect or consequential losses.
8. Cancellation Policy
We reserve the right to charge a cancellation/reschedule fee equivalent to 100% of the assessment cost if the cancellation occurs within 3 working days of the agreed-upon date or in the event of non-attendance.
The deposit of £150 will not be refunded if cancelled or rescheduled within 14 days of the agreed assessment date. A full refund will be provided if cancelled within 14 days of booking confirmation. Cancellations within this period will incur a £25 administration fee, resulting in a refund of £125.
All cancellation or rescheduling requests must be submitted in writing via email to rosie@dyslexiadiscovery.org.
9. Cancellation by Dyslexia Discovery
In the event of an emergency leading to the cancellation of an assessment, we will attempt to provide a minimum of 24 hours’ notice and reschedule the assessment, if possible.
The client is responsible for providing an emergency contact number when booking the assessment.
10. Payment Terms
If full payment is not received 7 days before the agreed assessment date, the assessment will be cancelled and rescheduled upon receipt of payment.
11. Changes to Terms and Conditions
We reserve the right to modify or update these Terms and Conditions at any time. The revised terms will be effective upon posting.
